The age and continual flexing of a belt will cause it to crack and deteriorate. Stretching loosens the belt, causing it to slip on the pulleys. A cracked belt will eventually break. The tension and general condition of the belt should be checked periodically.

If the straightedge does not lie flat, then one of the pulleys is misaligned. To validate which of the pulleys is causing the issue, check each against other pulleys until you isolate the one that is out of line. Pulley alignment can also be checked with a laser alignment tool.
